Tribal Home Visits Standard 650-25-70

Tribal home visits are periodic visits to isolated older individuals residing on an American Indian Reservation to monitor their health and well-being and identify service needs with an emphasis on referral and linkage to available services.
Tribal home visits must occur in the home and are available to individuals age 60 and older. Priority for tribal home visits must be given to older individuals:
- Residing in rural areas.
- With the greatest economic need (with particular attention to low-income older individuals including minorities with limited English proficiency).
- With the greatest social need (with particular attention to low-income older individuals including minorities with limited English proficiency).
- With severe disabilities.
- With limited English proficiency.
- With Alzheimer’s disease and related disorders with neurological and organic brain dysfunction and the caretakers of such individuals.
- At risk for institutional placement.
